Dental fear is a debilitating condition that affects many adults and children. It can originate from past traumatic dental experiences, physical or sexual abuse. The fear can result in someone not seeking dental treatment or regular check ups leading to untreated decay, gum disease, infection, broken teeth and severe pain.
Dental sedation can take the form of intravenous sedation, oral sedation and nitrous oxide sedation. All forms of sedation work on the central nervous system to produce feelings of calmness, reduce pain and anxiety. Some of the drugs use can also produce amnesia which can be very useful as there is little to no memory of the dental procedures performed. There is also a muscle relaxant effect with most of the sedative drugs used which helps to ease any muscle tension during and after the procedure.

Intravenous sedation involves administering sedative medications into the veins via a cannula inserted in the arm. The patient is sedated but is still conscious and is able to respond to verbal instructions. Importantly, the patient is able to breath on their own and maintain their airway reflexes. IV sedation is commonly referred to as “sleep” dentistry which is incorrect. If the patient is asleep, they would have achieved a very deep level of sedation or general anaesthesia which is very dangerous in a dental setting. The probable reason most patients feel they have been asleep during procedures because of the IV drug cause memory loss from the time the drugs are administered till the time the patient is recovered.

Oral sedation is normally achieved by administering the sedative drug orally in tablet or syrup form. The most common class of drugs administered are benzodiazepines.

This is one of the oldest forms of sedation in medicine and dentistry. it is also known as “laughing” gas as nitrous oxide has the ability to induce euphoria, muscle relaxation and has analgesic properties. Nitrous oxide and oxygen is administered through a mask. Nitrous oxide works quickly and induces a state of relaxation, euphoria and has a small degree of analgesia.

Virtually, all types of dental procedures can be performed when the patient is sedated. We often perform multiple procedures on our very anxious patients so that multiple problems can be addressed in the one visit. There will be instances where multiple visits will be needed due to the complexity of the problems that the patient presents with. Our priority is to keep the sedation procedure safe and comfortable for the patient.
This can only be answered by the sedationist performing a thorough assessment of the patient’s medical and dental history. As a rule, patients who want to undergo IV sedation will have to be classified as an ASA 1 or ASA2 patient. Generally healthy patients or patients with well controlled medical conditions are deemed suitable. However, we always conduct a thorough assessment of the patient’s physical, medical and dental condition before deciding whether someone is suitable for sedation dentistry. We may sometimes refer you to your GP for management of your medical conditions before proceeding with sedation dentistry, in particular IV sedation. Alternatively, nitrous oxide sedation is generally safe for almost all patients.
